About Short Term Care
After staying in a hospital, sometimes an in-between location is necessary in order to make the transition back to a home setting. A place offering skilled nursing services but at a less intense level than at the hospital. This is where short term care comes in.
Individualized Recovery Plan
Our care team works together to develop an individualized recovery program for every resident, which includes any therapies needed, such as occupational therapy, physical therapy and speech therapy. Our care plan includes out-patient therapy when available, to help make the transition home go smoothly while expediting recovery time.
